Social Care Practitioner

The Social Care Practitioner role is within the Integrated care team (LAP) which consists of Community Matrons, Mental Health Practitioners, Social Workers and Occupational Therapists. The LAP team involves supporting people with multidisciplinary support to look at the needs of the resident in a holistic way helping to prevent longer term challenges and admissions to hospital. Your role will involve: Completing Care Act Assessments with residents and their carers.

Holding and managing a case load. Assisting the duty function of the team, and providing advice and signposting to residents.
