You will manage a diverse caseload of individuals facing a range of challenges including physical and mental health conditions, disabilities, musculoskeletal issues, social isolation, housing and financial difficulties, caring responsibilities, and other factors affecting employment outcomes. Using a social prescribing and coaching approach, you will undertake holistic assessments, develop personalised work and wellbeing plans, coordinate access to community and voluntary sector services, and support individuals to achieve sustainable employment and positive life outcomes.

Contribute to delivery of WorkWell outcomes and key performance indicators. Work across community, primary care, workplace and virtual settings. Support integration of health, wellbeing, employment and community support services across the WorkWell pathway., Social Prescribing and Personalised Care Undertake holistic, person-centred assessments to identify health, wellbeing, social and employment-related needs.

Develop personalised support plans based on individual goals, strengths and aspirations. Support individuals to access community groups, voluntary organisations and local support services. Address wider determinants of health including housing, debt, social isolation, transport, digital exclusion and caring responsibilities. Empower individuals to increase independence, resilience and self-management.

Promote wellbeing, community connectedness and social inclusion WorkWell Coaching and Employment Support Manage a caseload of participants experiencing health-related barriers to employment. Undertake holistic biopsychosocial assessments to identify barriers and opportunities relating to work and wellbeing. Support individuals to remain in work, return to work or move towards sustainable employment.

Provide coaching, motivation and practical support to improve confidence, resilience and employability Support participants with CV development, job applications, interview preparation and skills progression Develop and review personalised work and wellbeing action plans Promote the positive relationship between good work, health and wellbeing Case Management and Care Coordination Act as lead practitioner for allocated participants.

Coordinate support across health, employment, skills, welfare and community services Facilitate warm handovers and supported referrals to partner organisations. Maintain regular contact with participants and monitor progress towards agreed outcomes. Identify and escalate safeguarding, welfare or clinical concerns in line with policy. Support participants through transitions such as returning to work, job loss, changes in health status or personal circumstances.

Partnership and Community Engagement Build and maintain relationships with community organisations, VCSE partners, local authorities, healthcare services and employment providers. Develop knowledge of local community assets and support pathways. Promote WorkWell and social prescribing services across partner organisations Work collaboratively with Occupational Health Advisers, Work and Health Coaches, Mental Health Practitioners and other MDT members.
