Join our team as a Social Services Coordinator, where you'll play a key role in supporting our residents' emotional, social, and discharge-planning needs. Working alongside our Social Services Director, you'll help ensure every resident receives compassionate, well-coordinated care throughout their stay. What You'll Do Partner with the Social Services Director to plan, implement, and evaluate our social services programs Collaborate with administration, medical, and nursing staff on social services planning Complete assigned resident assessments and MDS sections Help develop and revise resident care plans, working directly with residents and families Coordinate care plan meetings, care conferences, and discharge planning meetings Support discharge planning: DME orders, home health/caregiver coordination, transportation, and communication with resident representatives Liaise with attending providers on discharge timelines, status updates, and resident requests Help residents and families access financial, legal, and community resources, including Medicaid applications Maintain accurate, timely documentation in resident medical records Contribute to policy development and annual reviews Champion Resident's Rights and support dignity-centered care Support Advance Care Planning and ensure staff understand residents' code status and end-of-life wishes Conduct mood and behavior evaluations and coordinate related care plans Ensure residents facing mental illness or psychosocial challenges get access to appropriate resources Help residents voice and resolve grievances, reporting outcomes per facility procedure Participate in the facility's QAPI program Arrange mental health and psychosocial counseling services Coordinate end-of-life care, including hospice education and enrollment Assist with obtaining personal items and clothing for residents Support the facility's psychotropic medication program What You'll Need Bachelor's degree from an accredited college 1–2 years of experience in a healthcare clerical or staffing role (long-term care/nursing home setting preferred) Nice to Have Bachelor's degree in social work or healthcare 1+ year of experience in the medical field 1+ year of experience in a skilled nursing facility
