This position ensures that the patient’s biopsychosocial needs are adequately met through the completion of comprehensive behavioral health assessments and engaging the patient, caregiver(s), and care team in treatment planning and care coordination activities. The Social Worker is responsible for the provision of brief evidence-based behavioral health therapies and crisis intervention as a behavioral health expert within an integrated health care model.

Social Workers also work collaboratively with other members of the local care team and is key in ensuring access to equitable behavioral health care in alignment with the organization’s mission, vision, and values.This is an ideal position for a Social Worker who seeks an opportunity to contribute to the health and well-being being of highly complex patients, enjoys a collaborative multidisciplinary team-based approach to care, and is excited to engage in developing and nurturing our innovative, value-based clinical model focused on caring for patients with complex needs.ESSENTIAL DUTIES AND RESPONSIBILITIESSupports the behavioral health needs of patients on an assigned patient caseload in accordance with the established clinical practices of the behavioral health teamUses evidence-based models of care to deliver brief treatment for common mental health issues such as depression, anxiety, coping with loss, mood management, and substance-use disordersSupport psychotropic medication management by collaborating with prescribing providers, supporting treatment adherence needs, partnering with patients/caregivers for continuous monitoring of efficacy and potential negative side effects, and coordinating with internal psychiatrists and clinical care teams to address urgent psychotropic issuesParticipate in regularly scheduled caseload reviews and clinical team meetings; facilitate effective communication regarding patient's behavioral health needs among all members of the clinical team both verbally and in writingComplete and document assessment of patients in a timely mannerFacilitate treatment plan changes for patients who are not demonstrating improvement in symptoms by consulting with patient’s primary care provider/members of the clinical team and using measurement-based care to attune behavioral health interventions to the existing needs and symptom burden of patientsDemonstrate the ability to serve as a subject matter expert in a particular area(s) of behavioral health care and supports the behavioral health team at large by sharing knowledge and resources related to area of expertiseEnsure care is delivered in a manner that honors patient’s diverse values, beliefs, behaviors, backgrounds, social, cultural, and linguistic
