You see the medications on the table, the fall risks, the caregiver dynamics, the food insecurity, the missed follow-ups, and the small clinical changes that can become major health events if no one catches them early. Clover is building a model around that level of visibility. We are seeking a compassionate and driven Nurse Practitioner to join our growing Clover Home Care Team in New Jersey. This hybrid role supports Medicare Advantage members throughout Bergen, Burlington, Middlesex, Hudson, and Ocean Counties, combining in-home visits with telehealth care.
This is an opportunity to build meaningful relationships with patients while improving outcomes for medically complex and vulnerable senior populations. What You'll Do Conduct in-home and telehealth visits for Medicare Advantage members. Perform comprehensive assessments, medication reviews, and chronic disease management. Develop and implement individualized care plans that address both clinical and social determinants of health.
Identify barriers to care, including fall risks, food insecurity, caregiver challenges, and gaps in treatment adherence. Collaborate with physicians, care coordinators, social workers, and other interdisciplinary team members to deliver holistic, member-centered care. Leverage Clover Assistant and EMR technology to support proactive care management and close gaps in care. Monitor health status and intervene early to help prevent avoidable emergency department visits and hospitalizations.
Educate members and caregivers on disease management, preventive care, and treatment plans. Qualifications Active Nurse Practitioner (NP) license in New Jersey or ability to obtain licensure. National board certification as a Nurse Practitioner. Active DEA and New Jersey CDS registration, or ability to obtain. Experience caring for adult or geriatric populations in primary care, complex care, chronic care management, transitional care, home-based care, or value-based care settings.
Strong clinical assessment and chronic disease management skills. Passion for serving vulnerable senior populations. Comfortable utilizing technology and telehealth platforms to enhance patient care. Excellent communication and collaboration skills. Preferred Qualifications Home care, home health, home-based primary care, palliative care, geriatrics, or population health experience. Experience caring for Medicare Advantage members.
Prior experience in value-based care models. Experience managing medically complex and high-risk patient populations. Bilingual skills are a plus. Why Clover Health?
