Overview Resident Care Manager - RN Schedule: Monday-Friday 9am-5pm Salary: $80,000 - $90,000 / year DOE In this role, you'll lead the delivery of exceptional, person-centered care that helps residents achieve their highest level of health, independence, and quality of life. Working closely with the interdisciplinary team, you'll coordinate and oversee care for both short-term and long-term residents, ensuring services are individualized, clinically sound, and responsive to each resident's evolving needs.
As a Resident Care Manager, you'll play a key role in championing holistic care, driving positive outcomes, and creating an environment where every resident can thrive. How you’ll do it: Lead and inspire a high-performing nursing team while fostering a culture rooted in our mission, vision, and values. Advocate for resident rights and deliver resident-centered care that promotes dignity, independence, and quality of life.
Coordinate care across physicians, nurses, therapists, managed care partners, and the interdisciplinary team to achieve the best possible outcomes. Oversee admissions, assessments, care planning, care conferences, and discharge planning to ensure exceptional resident experiences. Monitor resident conditions, physician orders, incidents, and clinical documentation to maintain high standards of care and compliance.
Drive quality improvement initiatives, identify process enhancements, and support regulatory readiness. Partner with families and healthcare representatives to maintain open communication and collaborative care planning. Support MDS coordination, PDPM and OBRA compliance, utilization review activities, and reimbursement-related processes. Mentor and develop nursing staff through coaching, training, and accountability.
Provide leadership support, including on-call coverage, as needed. Perform other duties as assigned About you: Minimum of 1 year’s professional nursing experience in a skilled nursing setting. Registered Nurse (RN) license from an accredited program, school or university required. Prior supervisory experience in a similar setting including performance management, discipline and management of staff.
Demonstrated ability to work independently and make sound judgements concerning all aspects resident care. Understand state, federal and local regulations in relation to long term care and skilled nursing requirements. Current and valid CPR certification. Knowledge of electronic medical records systems and a general computer proficiency.
