Our mission for the next five years is to close the gap in the number of people missing out on what they need at the end-of-life.
Job Description About the service: Marie Curie is launching the Solihull Identification and Support Service, a targeted, place-based model providing proactive palliative and end-of-life care across six Solihull Primary Care Networks. The service will support earlier identification, stronger end-of-life register reviews, advance care planning, treatment escalation planning, and improved coordination across primary, community and specialist care pathways.
By moving from reactive crisis response to earlier community-based support, the service aims to improve outcomes and experiences for patients, families and carers, while contributing to system priorities such as better hospital flow and reduced avoidable urgent care use. About the role: We are looking for experienced and compassionate Clinical Nurse Specialists to help deliver this service, supporting earlier identification of people who may be approaching the last year of life and improving access to timely, coordinated, person-centred care.
Working as part of a multidisciplinary and multi-agency model, you will be embedded in primary care MDTs and support GP practices, community services and specialist palliative care partners to identify people earlier, review end-of-life registers, undertake holistic assessment, and support advance care planning, treatment escalation conversations and recording of patient preferences. The role is central to helping people, families and carers receive more planned, coordinated and responsive support closer to home.
This role offers the opportunity to help develop a new place-based service with a clear strategic focus: reaching people earlier, improving the quality and consistency of end-of-life care, strengthening partnership working with Solihull PCNs and local system partners, and using learning from the service to shape future development across Birmingham and Solihull.
