Advanced Practice Nurse - Home Based Primary Care

The Iowa City Veterans Affairs Health Care System (ICVAHCS) is seeking a Full-Time, Advanced Practice Nurse - Home Based Primary Care to join the Rehabilitation and Extended Care Service Line at the VAMC in Iowa City, Iowa. Note: This is an OPEN CONTINUOUS ANNOUNCEMENT through December 15, 2026. The first referral cut-off is October 11, 2026, at 11:59 p.m. EST, with monthly cut-offs on the 1st. Major duties: The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.

Major duties and responsibilities of the position: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team.

Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. All visits are conducted at patient's homes, might include assisted living facilities.
