This Registered Nurse - IVC Community Care Coordinator position is located in the Community Care Service located at the Augusta, ME location. This position is full-time at 40 hours per week. Major duties: Duties include but are not limited to: Coordinates timely access to community care services, including receiving, triaging, and directing Requests for Services (RFS) to ensure Veterans receive the right care at the right time.

Develops, implements, and manages individualized care coordination plans, ensuring accurate communication across VA and community providers. Serves as a liaison between VA services and community partners, ensuring seamless communication, scheduling, and follow-up. Provides Veteran-centered education and support by promoting self-management and incorporating patient values into care decisions. Reviews and tracks clinical documentation from community providers to ensure accuracy, completeness, and alignment with VA standards.

Collaborates effectively within the interdisciplinary team to resolve care barriers and improve Veteran outcomes. Applies and models evidence-based practice to enhance care quality, resource utilization, and performance improvement. Supports quality, safety, and patient experience initiatives to improve timeliness, satisfaction, and environment-of-care factors. Contributes to professional development by mentoring, sharing knowledge, and fostering a supportive team environment.

Ensures full compliance with privacy, security, and ethical standards, protecting sensitive information and upholding VA policies. Work Schedule: Full-Time, Monday - Friday, 7:30am - 4:00pm or 8:00am - 4:30pm Virtual: This is not a virtual position. Permanent Change of Station (PCS): Not Authorized Notifications: This position is an AFGE Bargaining Unit position This position is in the Excepted Service This position is covered by a special rate Veterans' preference does not apply for internal or other current permanent Federal agency employees.

Current and former Federal employees must submit copies of their most recent SF-50, (Notice of Personnel Action). The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted). In some cases, more than one SF-50 may be required to show a higher grade previously held. Pay: Competitive salary, regular salary increases, potential for performance awards Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Licensure: 1 full and unrestricted license from any US State or territory VA offers a comprehensive total rewards package: VA Nurse Total Rewards Requirements: U.S.

Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959.
