This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Major duties: VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU VA Careers - Licensed Clinical Social Worker: https://youtube.com/embed/U_xC25QsN0w Total Rewards of a Allied Health Professional The Grant and Per Diem (GPD) Program provides transitional housing with clinical and case management services, at times at the GPD grantee location, to homeless Veterans struggling with complex issues, including poor physical health, mental health issues, substance use, unemployment, financial problems, etc.

The major duties and responsibilities of the position include but are not limited to: Overseeing admissions into GPD, working in concert with community agencies and the HJCR Outreach Specialists. Monitoring timeliness pertaining to admissions into the GPD Program. Acting as a subject matter expert related to procedures and alignment with community resources that enable homeless Veterans to access the GPD program.

Be responsible for development and administration of the GPD program preadmission practices and procedures. Acting as a subject matter expert on GPD transitional housing and other HCHV grant opportunities available as housing resources for homeless Veterans. Ensuring best practices are advocated for and utilized, and help develop local policies and procedures related to the delivery of services. Monitoring, assessing, and providing oversight of the clinical care and compliance of the programs receiving per diem as described in the grantees current grant application (e.g.

Bridge Housing, Low Demand, Service Intensive, Clinical Treatment, & Hospital to Housing Models). Intervening to facilitate compliance or corrective actions and develops, facilitates, and monitors plans of correction to help Veterans maintain their housing. Coordinating with community agencies, organizations, and groups to learn of services or resources available for homeless Veterans as well as provides current information on services and benefits available in the VA for GPD Program participants.

Coordinating care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Providing crisis intervention when indicated including evaluation, assessment, and triage. Utilizing various treatment modalities of individual and group work, casework, community organization, and community outreach activities to alleviate or remove problems that interfere with the GPD participants' ability to achieve independence.

Working with multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to GPD Program Veterans. Providing leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for grantees. Demonstrating skills in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program areas.
