As a Community Support Specialist, you will provide community-based mental health services to adult clients through assessment, diagnosis, treatment planning, service coordination, and intervention implementation.

Develop and implement individualized treatment plans in collaboration with clients, families, and other stakeholders. Utilize evidence-based and strengths-based approaches that promote empowerment, resilience, and skill development. Coordinate care with therapists, primary care providers, community organizations, and other service partners. Facilitate PSR group sessions that support client treatment plan goals and overall wellbeing.

Facilitate ITCD group sessions as applicable to designated Community Support positions and client treatment goals. Build and maintain productive relationships with clients, support networks, and community stakeholders. Collaborate with interdisciplinary team members to ensure effective service delivery and continuity of care. Maintain timely, accurate, and compliant documentation of services, interventions, and client progress.

Participate in ongoing training and professional development activities to strengthen clinical knowledge and service effectiveness. Education, Experience, and/or Credential Qualifications: There are many ways in which you may qualify for this position... Bachelor’s degree in Psychology, Social Work, or other human services specialized field; OR... Bachelor’s degree in an unrelated field with two years of related work experience; OR...

Any four-year combination of higher education and two years of related work experience; OR... Associate’s of Applied Science in Behavioral Health Support from an approved academic institution OR... Four years of relevant qualifying experience in delivering services to individuals with mental health disorders, substance use disorders, or developmental disabilities.
