POSITION SUMMARY: The Intake Coordinator is responsible for performing all steps of the patient intake and referral process. This role is essential for ensuring that every patient referred to the agency is accurately vetted, documented, and approved for services in a timely manner.
ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING: • Serves as the primary contact for referral reception from hospitals, physician offices, and other sources, accurately gathering all necessary demographic and clinical information. • Creates the patient shell and initial patient record within the Homecare Homebase (HCHB) EMR system. • Performs thorough checking of patient eligibility for home health or hospice services against regulatory and payer criteria, ensuring the patient meets the requirements for home health or hospice.
• Conducts comprehensive insurance verification and benefit checks for all potential patients to confirm coverage and benefits. • Initiates and tracks the requesting of initial insurance authorization for services, communicating with payers until approval is secured. • Communicates effectively with clinical teams, referral sources, and physicians to obtain missing documentation and clarify patient needs or service eligibility.
• Ensures all aspects of the intake process are compliant with federal, state, and payer rules and are completed within required timeframes. • Other appropriate services and special projects as assigned. QUALIFICATIONS & REQUIREMENTS FOR THE POSITION: • At least one (1) year of experience in a healthcare office setting, such as admissions, patient access, or intake, with experience in home health or hospice preferred. • Demonstrated experience or ability to quickly learn and utilize an Electronic Medical Record (EMR) system; familiarity with Homecare Homebase (HCHB) is a major plus. • Basic knowledge of Medicare, Medicaid and commercial insurance plans and terminology related to eligibility and authorization.
• Strong understanding of medical terminology and documentation requirements. PREFERRED KNOWLEDGE AND SKILLS: • Excellent time management and organization skills. • Strong problem-solving skills to resolve insurance and documentation issues. • Ability to work effectively in a high-volume, fast-paced environment and prioritize multiple tasks. • Exceptional attention to detail. • Demonstrated ability to manage multiple complex cases simultaneously.
