To Apply for this Job Click Here Job ID: 897392 Estimated Weekly Gross: $1,911.81 Job Title Registered Nurse – Case Management (Hospital) Location Atlanta, GA | Acute Care Schedule / Job Type Contract | Days | Monday–Friday | 5×8-Hour Shifts (8:00 AM – 4:30 PM) | No Weekends | No Call Key Responsibilities Assess, evaluate, and drive individualized discharge planning for each patient Coordinate services to transition patients to the appropriate level of care Initiate, evaluate, and re-evaluate treatment plans on a daily basis Monitor length of stay and proactively identify and address barriers to discharge Follow patient progress daily and adjust care coordination strategies accordingly Adhere to Medicare regulatory guidelines regarding patient choice, IMM notices, and MOON requirements Required Qualifications Active Georgia RN license or valid Compact RN license required at time of submission Minimum of 3 years of case management experience required; first-time travelers will not be considered Strong knowledge of discharge planning, care coordination, and utilization management Ability to complete all compliance documentation by the Tuesday prior to start date All requested time off (maximum 7 days per contract) must be finalized at time of submission; no schedule changes or RTO requests will be accepted post-offer or on-site Local candidates are welcome and accepted Preferred Experience / Skills Prior travel nursing experience in a hospital case management setting Familiarity with Medicare/Medicaid regulatory requirements and compliance standards Experience working in a fast-paced acute care environment with high patient volumes Why Join Us Monday–Friday day shift schedule with no weekend or on-call requirements, offering a strong work-life balance Opportunity to work in a well-established acute care setting with a structured onboarding and orientation process To Apply for this Job Click Here
