Evaluate, diagnose, and manage acute and chronic medical conditions. Conduct physical examinations and wellness visits. Order and interpret diagnostic tests and laboratory results. Develop evidence-based treatment plans. Prescribe medications and therapies within scope of practice. Coordinate referrals and follow-up care with specialists. Educate patients on disease prevention and healthy lifestyle choices.
Maintain accurate and timely electronic medical records. Collaborate with physicians, nurses, and interdisciplinary healthcare teams. Qualifications Required Graduate of an accredited Nurse Practitioner or Physician Assistant program. Active California NP or PA license, or ability to obtain licensure. Strong clinical assessment and patient management skills. Excellent communication and interpersonal abilities.
Preferred 1+ year of experience in Family Practice, Primary Care, Internal Medicine, Community Health, Urgent Care, or Outpatient Medicine. Experience managing chronic diseases and preventive care services. Bilingual skills are a plus but not required.
