You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high quality home health services.Location: San Diego North Central, Coastal and CentralScheduled: Monday-Friday with Weekends RotationResponsibilities • Provide nursing care according to the RN developed Plan of Care.
• Monitor and document changes in patient condition; report to RN/physician as needed. • Assist with activities of daily living to promote patient independence. • Educate patients and caregivers within LPN/LVN scope of practice. • Maintain accurate EMR documentation. • Participate in case conferences and coordinate care with the home health team. • Follow infection control, HIPAA, and company policies.
• Current LPN/LVN license in the state of practice. • Graduate of an accredited practical/vocational nursing program. • Minimum 1 year acute care experience preferred. • Current CPR/BLS certification. • Valid driver’s license, reliable insured vehicle. • Strong communication, clinical judgment, and ability to work independently.Qualification • Current LPN/LVN license in the state of practice. • Graduate of an accredited practical/vocational nursing program.
