Fox Subacute - Mechanicsburg is seeking a dedicated and compassionate Social Worker to join our team in Mechanicsburg, PA. This role offers an opportunity to make a meaningful impact on the lives of our patients by providing emotional support, resource coordination, and advocacy within a subacute care setting. The Social Worker will collaborate closely with multidisciplinary teams to ensure comprehensive care and support for our patients and their families.
Responsibilities Conduct psycho-social assessments of patients and their families to identify needs and provide appropriate support. Develop and implement individualized care plans in collaboration with the healthcare team. Advocate for patient rights and access to community resources. Provide counseling, crisis intervention, and emotional support to patients and families. Coordinate discharge planning and facilitate referrals to community services and support networks.
Maintain accurate documentation and comply with all regulatory and facility policies. Requirements Bachelor’s or Master’s degree in Social Work from an accredited program. Valid license or certification as required by the State of Pennsylvania. Previous experience in a healthcare or subacute care setting preferred. Strong communication, interpersonal, and problem-solving skills. Ability to work effectively both independently and as part of a multidisciplinary team.
