🩺 What You'll Do Conduct skilled nursing visits in patients' homes (assessment, reassessment, medication administration, IV therapy, and treatments) Develop and implement individualized care plans Educate patients and caregivers on managing conditions and medications Collaborate with a multidisciplinary team to ensure continuity of care Document visits and patient progress accurately and timely ✅ What You'll Need Current Registered Nurse (RN) license in the state of Pennsylvania.
